Chris joined the practice In 2001, as an associate and became Dr. Blazek's partner in 2003.  Together, with our exceptional team, we have carried on the tradition of relationship-based comprehensive care for each patient we serve.  Our unique approach to patient care has allowed us to grow our practice over the years with very little advertising.  Over 95% of our new patients are referred to us from our current patients.  This word-of-mouth referral system started with Dr. Dellios' first patient and has continued since 1956.  We look forward to serving you and your family for generations to come!

Dr. Collingwood has been recognized in both Milwaukee Magazine and Milwaukee lifestyle magazine. He has been involved as a volunteer with Mission of Mercy, donating dental service to provide treatment at no charge to those that cannot afford it.

He also studies continuing education courses at the Pankey Institute, Dawson Center, and Spear Center to stay current with the changes in technology, materials and techniques.

Our financial coordinator Sally will work hard to maximize your insurance coverage to help make your treatment affordable. If you do not have insurance Sally will assist you with payment options so you can get the treatment you need. Deb, our scheduling coordinator will work with you to make sure your appointment fits into your busy schedule. Together they have over fifty years of experience in dentistry!
